Graciosa is the northernmost island of the central group of islands, being located 36 km northeast from São Jorge and 57 km northwest from Terceira. The island has an area of 61 km², measuring 12.5 km in length and with a maximum width of 8.5 km. Low-lying and flat in the northern and northeastern areas, it rises gradually until the altitude of 398 m is reached at Pico Timão, located at the centre.

Off the east and southeast coasts are several smaller islets the largest amongst which are the 9 ha Iléu da Praia (off the coast of Praia), and the 32 ha Iléu de Baixo (off Ponta da Restinga). Madeiran storm petrels (Oceanodroma castro) breed on these islets as well as on Vila, off Santa Maria.
