The 128 km² island of Shiashkotan (Остров Шиашкотан) is located at the southern end of the Northern group of islands, 29 km southwest from Kharimkotan and 8 km southeast from Ekarma. The island consists of two distinct lobes connected by a narrow isthmus. Shiashkotan is mountainous — rising to a height of 944 m — with a rocky and often steep coastline. Offshore are dozens of small islets and emergent rocks, particularly on the eastern side.
The island is formed by two volcanoes: Kuntomintar in the south and Sinarka in the north. The isthmus connecting them averages between 85 and 125 m in height with a width of around 1 km.
